Severe deformity in patients with end-stage ankle arthritis has long been considered a relative contraindication for total ankle replacement. However, advancements in surgical techniques and a better understanding of the importance of correcting foot deformity in conjunction with ankle deformity have led to satisfactory outcomes in this group of patients. (SBQ12FA.62) A 60-year-old woman presents with the condition seen in Figure A following corrective forefoot surgery 6 years ago. She has attempted to manage her condition with shoe wear modifications, taping and splinting but has consistent pain. Background: We evaluated the compensatory change in ankle alignment due to knee malalignment and its relationship with varus knee deformities, as well as sex differences in compensation. Methods: From October 2016 to September 2017, 103 patients with end-stage knee osteoarthritis underwent primary total knee arthroplasty (TKA). Ninety-five knees (78 patients) were included. It is determined by the distal part being more medial or lateral than it should be. Whenever the distal part is more lateral, it is called valgus. Whenever the distal part is more medial, it is called varus. In the latter, the varus deformity represents the beginning Ankle Varus Deformity. - Discussion: - varus deformity of the ankle may follow distal tibial fracture or distal tibial epiphyseal frx; - deformities due to distal tibial epiphyeal injuries are often progressive; - deformities due to tibial plafond injuries are usually static (from malreduction at the time of the original procedure); Severe flatfoot and hindfoot valgus deformity may present with lateral ankle pain in the region bounded by the anterior fibula and the sinus tarsi . This lateral ankle pain has been attributed to extraarticular lateral hindfoot impingement including talocalcaneal impingement (between the lateral talus and calcaneus) [ 61 ] and subfibular impingement (between the calcaneus and fibula) [ 62 – 64 ] (Fig. 12A , 12B , 12C ). The varus deformity can be quantified with the hip-knee-ankle angle (HKA), which measures the angle between the mechanical axis of the femur and the center of the ankle joint on AP, full-length, weight-bearing radiographs. A decreased LDTA represents a varus deformity. The tibial–talar angle (Fig. 9.2 c) is defined by the tibial and talar articular surfaces of the ankle joint. When the tibial–talar angle is >10°, the joint is defined incongruent (unstable) . The calcaneal–tibial alignment (measured on the Cobey/Saltzman view) is useful to confirm any varus or valgus deformities as well as to assess every talar compensation (inversion and eversion) to an abnormal LDTA.
